Each variant adds only what's genuinely different in that frame's documentation — a place for automatic thoughts in CBT notes, a target-hierarchy field in DBT notes, a process-label checkbox for ACT notes, parts-language fields in IFS-informed notes, SUDS / VOC tracking in EMDR-adjacent notes. The clinical work is yours; the structure is the help.

These are administrative scaffolds, not clinical protocols. The trademarked sequences (EMDR's eight phases, IFS's Self-leadership steps, DBT's skills chains) are intentionally not reproduced — the modality names are categories, not technique transmission. The pack is also not HIPAA-compliant on its own; compliance depends on where you store completed notes (a properly-configured EHR or a Notion workspace on the Business plan with a signed BAA).
